If you ordered a subscription for Museum or Exhibition and did not receive one or more issues, please submit a claim by completing the information in the fields below.

  • You have 6 months to submit a claim.
  • To help expedite your claim, complete as many fields as possible.
  • You may claim multiple issues on one form
  • If you manage subscriptions for more than one organization, please submit separate claims for each organization reporting missing issues
